Here is the power supply. You can clearly see the black soot on the capacitor below. This is from "gassing".
Not a good sign. Since this is a "rail-to-rail" opamp design, I replaced the zener diodes as well as the
capacitors. The zener diodes control the voltage on the rails and it is important that the positive and negative
rails have close tolerance.
